Heart Meals ("we," "us," "our") operates heartmeals.nyc and provides medically tailored meal and grocery delivery coordination for eligible New York Medicaid members. This Privacy Policy explains what information we collect, how we use it, and the choices you have.

When you call us, fill out our eligibility form, or request a callback, we may collect:

Name, date of birth, and address
Phone number and email address
Health-related information you choose to share (such as a qualifying condition or dietary needs) so we can build your menu and confirm eligibility
Language preference

We do not collect information from our website through cookies beyond what is needed to operate our lead and callback forms.

To confirm your eligibility and connect you with the applicable Social Care Network (SCN), health plan, or care manager
To schedule, prepare, and deliver your meals and groceries according to your preferences and needs
To contact you by phone, email, or text message about your enrollment, delivery schedule, or in response to a question you asked us
To improve our service and respond to inquiries

Text messaging (SMS) & mobile information

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties, except as required to provide the SMS service itself (such as our SMS delivery platform) or as required by law.
